Nehalem Bay Health Center & Pharmacy recently welcomed representatives from the Oregon Health Authority for a Patient-Centered Primary Care Home site visit.

The visit provided an opportunity to demonstrate how NBHCP’s care model works in practice and how staff support patients through primary care, behavioral health, dental care, pharmacy services, school-based health, outreach, and care coordination.

NBHCP is proud to hold Tier 5 Patient-Centered Primary Care Home recognition, the highest level awarded by the state. This recognition reflects strong standards for access to care, whole-person services, care coordination, quality improvement, and patient engagement.
