We do our best to accommodate same-day appointments for urgent needs. Call us early in the day and we'll find the earliest available time.

A Photo ID: Any form of picture identification, such as a driver’s license or work ID, will do.
Medications and Supplements: A current list of all medications and dietary supplements you are taking.
Medical History: Information about past and current health conditions, surgeries, family health history, and immunization records.
Health Insurance Card: If you are insured, please bring your current insurance card(s).
Co-payment: If your insurance plan requires a co-payment, it will be collected at your visit.
