Connected Care Community Health Worker
Position Overview
Nehalem Bay Health Center & Pharmacy, located on the beautiful North Oregon Coast, is hiring a Connected Care Community Health Worker. Bilingual English/Spanish skills are preferred.
The Connected Care Community Health Worker provides information, resources, and care coordination to older adult patients and their families. This position completes home visits and Connected Care Protocols, supports patients with Advance Directives, helps with medication management, identifies fall hazards in the home, and connects patients and families with community resources.
The Connected Care Community Health Worker works closely with patients’ primary care providers and communicates key information about what matters to older adult patients in their life and care.

Duties
- In-Home Care, Assessment and Referral
- Complete the Connected Care Protocols through a series of home visits.
- Assess the health and social needs of clients using the Connected Care tools.
- Elevate concerns to the patient’s primary care provider or appropriate clinician.
- Collaborate with healthcare clinicians to develop care plans and make appropriate referrals for medical, social, and support services.
Outreach and Education
- Provide culturally competent health education and information to patients, families, and caregivers related to healthy aging and the 4Ms.
- Ensure patients are aware of available community resources, make appropriate referrals, and assist patients in completing applications and accessing resources as appropriate.
Patient Advocacy
- Advocate for clients within the healthcare system to ensure they receive appropriate care and services in line with what matters most to them.
- Support patients in understanding their rights and responsibilities as healthcare consumers.
Crisis Intervention
- Offer emotional support and crisis intervention for patients facing health-related challenges.
- Connect patients to mental health services or crisis hotlines as needed.
Documentation and Evaluation
- Maintain accurate records of patient interactions, services provided, and outcomes in the clinic’s Electronic Health Record system.
- Submit required Connected Care evaluation forms for all referred patients.
- Participate in evaluation surveys and meetings to continuously improve the program.
Community Building
- Participate in regional networks to share with and learn from other Community Health Workers.
- Collaborate with local organizations and community leaders to identify and advocate for improved older adult services.
- Perform other related duties as assigned.

Qualifications
- High school diploma or equivalent.
- Valid driver’s license and insured vehicle.
- 90-hour Community Health Worker certification, or ability to become certified.
- Minimum of two years of experience in a health or social service-related field preferred.
- Experience with and demonstrated commitment to serving the older adult population.
- Strong interpersonal and communication skills.
- Empathetic and culturally sensitive approach to working with diverse populations.
- Knowledge of local healthcare resources and social services, especially for older adults.
- Proficiency in data collection and record keeping.
- Ability to work independently and as part of a team.
- Familiarity with medical terminology and the healthcare system, including Medicaid, Oregon Health Plan, and Medicare.
- Bilingual and/or bicultural skills preferred.
Physical Requirements
- Ability to communicate both in person and by phone.
- Ability to lift up to twenty-five pounds.
- Ability to sit for long periods of time.
- Manual dexterity when inputting computer information.
- Exposure to normal noise, temperature, dust, and fume levels.
